Crisis Intervention

Our crisis intervention team visits with clients by telephone or face-to-face to assist them with the following:

• Development of individualized care plans addressing health, housing, safety needs and referrals to appropriate service providers
• Problem-solving, conflict resolution, harm reduction, relapse prevention, medication management and other life skills as appropriate (cooking, cleaning, accessing public transportation etc.)
• Completing applications for Canada Pension Plan (PP), Ontario Disability Support Program (ODSP), Ontario Works (OW), passports and other identification processes
• Supportive counselling
• Self-advocating for effective solutions
