FEEDBACK

We are interested in continuous improvement as an organization, and we learn through feedback. We also want the opportunity to correct our mistakes when we can. We encourage you to let us know when you’ve had an experience with our organization that did not meet your expectations, or that you believe could have been better.

You are welcome to provide feedback by:

  • Speaking to a staff member directly, in person or on the phone
  • Leaving a written note with a staff member
  • Asking a staff member to write down your complaint for you, and allowing you to review and sign it
  • Sending an email to a staff member
  • Completing a Feedback Form for a particular program (some programs such as Respite have a dedicated form that program staff will provide)
  • Completing this Feedback Form

It is always helpful whenever possible, when sharing feedback with us, for you to:

  • Provide your name
  • Share it promptly, before too much time has passed
  • Provide enough details for us to investigate
  • Let us know if you are expecting a particular outcome
  • Share your contact information for follow up

We will do our best to listen, investigate, and respond with an explanation or a potential solution to your feedback.

An investigation might include:

  • Internal information gathering
  • Asking you for additional information
  • Asking others who may have information relevant to your feedback

We will not disclose your identity as someone who has provided feedback to us, except to staff where relevant, without your permission.

If you are unhappy with our treatment of, or response to, your feedback, you may share further feedback with any member of management (if you have not already been in touch with a manager), or our Executive Director. Finally, you may share further feedback with our Board Chair.
